Patients increasingly expect their health to be tracked beyond hospital visits, while providers rely on continuous, accurate data to enhance decision-making. Also, elderly patients, hospice care patients, and those with terminal illnesses often need their vital signs to be regularly monitored through home care services or specialized care centers. RPM fulfills this need by using connected devices that deliver real-time health insights directly to healthcare professionals and providers.
When integrated with Care Management, RPM enables proactive interventions, reduces dependency on patient-reported data, and enhances patient health outcomes. This case study demonstrates how our RPM solution redefined care delivery for our client.
CLIENT
Our client is a US-based care management service provider serving hospitals and medical practices that want to shift from outdated manual data collection to automated, real-time monitoring. Their goal was to improve patient safety, reduce hospitalizations, and enhance engagement using connected devices and integrated technology.
Challenges
- Inaccurate Patient-Reported Data: Patients often misreported or forgot to log vital information, which made care decisions unreliable and sometimes delayed.
- No Real-Time Visibility: Care managers had no way to see vital changes (such as sudden blood pressure spikes) until the next patient check-in, with an increased risk of handling emergency situations.
- Time-Consuming Manual Data Entry: Data had to be manually entered into multiple systems, increasing errors and slowing down workflows.
- Inability to Prioritize High-Risk Patients: Without automated alerts, care teams struggled to identify which patients needed immediate attention.
- Low Patient Involvement: Patients interacted with care teams only during visits, leaving the patients disengaged from daily health management.
SOLUTION
We built a patient-centric mobile app integrated with connected health devices, powered by Validic. The solution allowed healthcare teams to capture and act upon health data continuously:
- Device Integration: The app connected with devices such as Withings blood pressure monitors, Fitbit wearables, and Omron glucose meters.
- Automatic Data Capture: Vital signs like blood pressure, heart rate, blood glucose, temperature, and sleep patterns were automatically recorded.
- Seamless Data Flow: Data is transmitted securely into the Care Management Portal, ensuring instant availability to care managers.
- Real-Time Alerts: Notifications were triggered for abnormal readings, alerting both patients and care managers.
- Patient Engagement: Patients used secure chat and email to communicate with care managers, ensuring continuous interaction.
